Rhinoplasty Without Surgery?

For very minor corrections, a rhinoplasty is possible without surgery. Your nose is corrected using injectables.

An abnormal shape of the nose cannot usually be corrected with fillers or injectables.

Sometimes there is a minimal bump on the top of the nose, for which a filler can be a solution.

However, you will have to repeat this treatment regularly. In that case, filler can be injected just above and below the bump, making the bump less visible.

Types of Rhinoplasty

Nose surgery is highly individualized, and the exact techniques a cosmetic surgeon will use during a procedure will look a little different each time, depending on a patient’s anatomy and desired changes. In general, however, a cosmetic surgeon’s techniques will fall under one of the following procedural approaches.

Open Rhinoplasty

For an “open” rhinoplasty, a small incision is made under the nasal tip, between the nostrils. This approach allows the cosmetic surgeon more complete access to the nasal structures, where he or she can then sculpt the nose into the desired shape. Because of this increased access, cosmetic surgeons will typically use the open rhinoplasty approach for patients requiring more extensive reshaping. The resulting scars are well-hidden in the natural contours of the nose so they are as inconspicuous as possible.

Closed Rhinoplasty

For a “closed” rhinoplasty, incisions are made inside the nostril. This approach is most commonly used for patients who need more minor adjustments to the nasal structures to achieve their desired improvements.

The advantage of closed rhinoplasty is that there is no visible scarring after surgery; however, this approach may not be suitable for more extensive rhinoplasty procedures.

Tiplasty

A “tiplasty” refers to the area of reshaping, rather than the incision techniques. As you might have guessed, tiplasty involves reshaping the nasal tip only, without affecting the other nasal structures. Cosmetic surgeons may use the open or closed approach for tiplasty.

Septorhinoplasty

Straightens the nose to correct a deviated septum and improve airflow whilst improving the overall appearance of your nose. Incisions are made in the internal lining of your nose to access the cartilage and bone and make any corrections to the shape of your septum (the structure that internally divides your nose into left and right halves).

Don't APPLY COSMETICS

Anesthesia

Rhinoplasty is an outpatient procedure that's performed under general anesthesia, by our registered anesthesiologist.

Making the Incisions

The first step with rhinoplasty is making the incisions that give access to the nasal tissues. With closed rhinoplasty, all incisions are made inside the nose. With open rhinoplasty, your doctor makes a short incision across the tissue that separates the two nostrils (the columella).

Grafting With Cartilage or Bone

Your doctor may also perform cartilage grafting, using nasal bone or cartilage from another site to straighten a deviated septum, build up cartilage, or change your nasal structure.

Splinting the Nose

Once your nose surgery is complete, your doctor will place a splint on your nose to allow everything to heal properly.

Risks

As with any procedure, there is a small risk of bleeding, infection or impaired wound healing with rhinoplasty. This is usually not serious and can easily be remedied.

After Surgery

After surgery, most patients have minimal to no bruising as our doctor uses tissue preserving and minimally invasive approaches. You will have a light splint on your nose for a about a week. Most patients experience little to no pain after surgery.

Aftercare Tips

Change Gauze

The internal dressings and splint that the plastic surgeon places will remain untouched for up to a week after your nose job. During this time, all you have to do is replace the piece of gauze, sometimes called the "drip pad," that's placed below your nose.

Avoid Strenuous Activity

The most important thing you can do in recovery is to relax. Avoid strenuous activity until Dr. Castillo gives you the green light to begin exercising again.

Control Your Tendencies

It's natural to smile, laugh, blow your nose, move your upper lip when brushing your teeth, and pull your clothes over your head. However, all of these activities can disrupt your splint. Be mindful of these impulses to avoid disrupting incisions.

Recovery

Rhinoplasty is performed as an outpatient procedure. In other words, you will be able to return home on the day of your surgery.

You should plan on resting during the first day following the procedure with your head elevated and use of cooling gels or ice to minimize swelling.

It is natural and normal for some degree of swelling and bruising to appear immediately after the procedure.

You may expect to resume your normal activities within one week.

You should continue to avoid any contact sports or strenuous activities for at least 3 weeks after surgery.

During this period, your doctor and his staff will follow up to ensure that you are experiencing a smooth recovery.

If necessary, your doctor will prescribe medications to relieve any potential discomfort.

Most bruising will resolve 7-10 days after surgery. A majority of the swelling will settle by the 6-week mark, fully concluding within 3 months. The overall shape of the nose will gradually improve over a year.

As swelling subsides, your striking results will continue to emerge. Once your new nose settles, you can expect it to bring you confidence and satisfaction for many years to come.

Do I have to go under anesthesia during a rhinoplasty?

Almost all rhinoplasties take place under anesthesia. Only in exceptional cases, for example, can the nasal tip be corrected under local anesthesia.

Is a rhinoplasty painful?

The saying goes that if you want to be beautiful you have to suffer pain. Fortunately, this does not apply to a rhinoplasty. Because you are anesthetized, the procedure itself does not hurt. You will hardly experience any pain after the procedure, but do take it easy. However, you may experience discomfort due to swollen eyes (sometimes the eyes are completely closed) and due to blocked nostrils.

How long will it take for everything to heal?

The end result after a rhinoplasty can often only be properly assessed after 12 to 18 months. Recovery generally takes a little longer than with other procedures. In addition to the skin, bone and cartilage also need time to recover. On average, it takes 6 weeks for the scar to heal properly. It takes a year to fully mature.

Can I wear glasses?

No, you are not allowed to wear glasses for the first 6 weeks after rhinoplasty.

When can I return to work after a rhinoplasty?

After a rhinoplasty, you can resume light work after one week. You should not exercise or do any physically demanding work for the first 4-6 weeks. After six weeks you can start exercising slowly. Depending on the extent of the operation and the progress of healing.

What do the scars look like after rhinoplasty?

The plastic surgeon performs the procedure through the nostrils. Sometimes an incision at the lower edge of the nasal septum is necessary. Or at the transition from the nostrils to the lips. Whichever method you choose together with the surgeon, you will hardly see anything. The scars may be temporarily red and swollen after the procedure. That is normal.

No, nasal tampons are not always necessary and depend on the type of rhinoplasty you have had. The nasal tampon may absorb moisture and blood. You can only breathe through your mouth. If you have to sneeze, do so with your mouth open.

What's the difference between 'open' and 'closed' rhinoplasty?

Open rhinoplasty involves your Cosmetic Surgeon making a small incision in the columella (the skin that covers the part of your nose that separates the nostrils), external incision allows the Surgeon to lift the skin and precisely alter the size and shape of the cartilage underneath.

‘Closed’ rhinoplasty is a surgery that is performed via small incisions entirely inside your nose.

Is there a lower or upper age limit for the rhinoplasty procedure?

As an ethical cosmetic surgery provider, Marmara Health Clinic does not perform cosmetic surgery on people under 18.

Cosmetic surgery is not usually recommended for patients who are not fully grown, as the face and body can change dramatically during adolescence.

We do not set an upper age limit for nose reshaping surgery. However, as with all patients, we assess your medical history and current health to make an informed decision on whether or not to perform the procedure.

Is rhinoplasty permanent?

Nothing in life is permanent, but your nose will enjoy the benefits of rhinoplasty for decades to come. Your skin, muscle, and soft tissue will continue to mature as you age. Rhinoplasty provides long-lasting structure to fend off the majority of aging concerns.

Will there be a visible scar after surgery?

With either the open or closed technique, there are no visible scars when the nose is fully healed.

How long does the surgery take?

Usually anywhere from 1.5 to 3 hours. All patients are then discharged safely to their home, a local hotel or aftercare facility.

Will I need to do any blood work or test before surgery?

Yes. All patients must do required blood work. If you are over age 40, additional tests may be required depending on your medical case (including Chest X-rays, Electrocardiograms, etc). This is all based on the individual patient and their health history.

What is recovery like?

Most patients have minimal to nose bruising or pain after surgery. On postoperative day 6 or 7, the cast is removed and you may resume normal social activities but not exercise.

Can my breathing be improved with rhinoplasty?

Yes. our doctor is uniquely trained in addressing any sinus or breathing issues at the same time as he improves the appearance of the nose. He is able to open the sinuses, straighten the septum and reduce the turbinates if needed.

Where are the incision made for rhinoplasty?

Most incisions are confined to the inside of the nose and are therefore not visible. In the “open” approach a small geometrically concealed incision under the nose is required. In the thousands of cases done we have never had an issue with the incisions being visible.

When can I exercise after surgery?

To prevent bleeding and increased swelling, avoid extreme physical activity. After 24 hours, you are encouraged to go for light walks and strolls. Do not engage in any heavy cardiovascular activity (running), any activity that involves straining (weight lifting) or anything that involves bending over (yoga, pilates) for at least three (3) weeks, unless otherwise instructed.

When are the stitches removed after rhinoplasty?

The stitches are typically removed within seven to fourteen days after surgery.
